Patient Information: Preparing for Your Visit in Bellevue
Welcome to CarePlus Foot and Ankle Specialists. We believe informed patients are the most successful patients. Whether you are seeing Dr. Hubert Lee for a routine check-up or treatment for heel pain or ingrown nails, here is everything you need to prepare for your visit.
CarePlus is accepting new patients. Our Bellevue clinic is conveniently located for patients in the Bel Red and Spring District areas. To schedule, call our office at (425) 455-0936 or book online. We also offer same-day and urgent care appointments for time-sensitive foot and ankle problems.
Please arrive 10 minutes before your appointment to complete registration, or download and fill out the new patient forms in advance. Bring the following with you:
- New Patient Forms
- Driver’s license or state ID
- Insurance cards
- A list of allergies and current medications
- Any relevant previous medical records, including x-rays
If you are unable to keep your appointment, please let us know as soon as possible so we can offer that time to another patient who needs care. Cancellations with less than 24 hours’ notice, and missed appointments, are subject to a $50.00 fee.

Billing and insurance FAQs
How do I pay my bill, and what payment methods do you accept?
Payment for all copays and services is due at the time they are rendered. We accept cash, checks, debit, Visa, MasterCard, and American Express. For in-network plans, we will bill your insurance, though payment remains the patient’s responsibility. You can pay your bill by calling our billing service at (866) 514-0380 between 8:00 am and 3:00 pm, Monday through Friday, or pay online. Your account number is on your statement.
Do you see self-pay patients?
Yes. We see self-pay patients. Because the cost depends on what your visit involves, call our office at (425) 455-0936 and we will give you an estimate before you are seen.
What insurance does CarePlus accept?
