We are currently accepting new patients. Please call our office at (425) 455-0936 to schedule an appointment, or schedule an appointment online. If you are unable to keep your appointment, please inform us as soon as possible so that we may use that time to see another patient in need of care.

We request payment for all services at the time they are rendered. We accept cash, checks, Visa, MasterCard, American Express, and Discover. If we are a participating provider, we will bill your insurance company. However, payment is the patient's responsibility.

Please arrive early to your appointment to complete the registration process. In addition, please bring the following:
- Completed new patient forms (see below for downloadable forms)
- Driver's license or state ID
- Insurance cards
- List of allergies and current medications
- Any relevant previous medical records
To help us comply with the Red Flags Rule created by the Federal Trade Committee that protects consumers from identity theft, we ask that you bring a valid ID along with your insurance card. If you do not have a valid ID, we may not be able to accept you as a patient.
