Cancellation and No-Show Policy
If you are unable to keep your appointment, please inform us as soon as possible so that we may use that time to see another patient in need of care. If you cancel less than 24 hours in advance or do not show up to your appointment, there will be a $40.00 fee.
New Patients
We are currently accepting new patients. Please arrive 10 minutes prior to your appointment time to complete the registration process. You may also download and fill out the new patient forms prior to your visit. Please bring the following to your appointment:
- New Patient Forms
- Driver's license or state ID
- Insurance cards
- List of allergies and current medications
- Any relevant previous medical records
Payments
We require payment for all services at the time they are rendered. We accept cash, checks, debit, Visa, MasterCard, and American Express. If we are an in-network provider, we will bill your insurance company. However, payment is the patient's responsibility.
